Transition Planning Beyond School

As our youngsters with special needs develop, it is essential that they have

access to all of the resources available to them and their families. Aside from

working with your case manager to develop a transition plan, there are many

“outside of school” agencies that provide services to children and adults with

special needs beyond school. Below is a list of links to assist in this transition.
